Biography
Daisy Jacobs is an Advanced Clinical Practitioner at St Nicholas Hospice Care, Bury St Edmunds, with a passion for improving palliative care. Daisy is commitment to applying theory to practice and role models the concept of lifelong learning as a registered nurse. She has been awarded an NIHR ARC Impact Fellowship to further develop the Families Administering Medications (FAM) programme, which enables trained family carers to administer symptom control medications at home. Daisy's project will broaden awareness and further implementation of the FAM programme across Suffolk and North East Essex Integrated Care System. She aims to refine, embed, and expand the initiative to enhance patient choice and dignity in end-of-life care. With a background in palliative care research and a master's degree, Daisy is developing her knowledge mobilisation skills as an emerging healthcare leader. Her fellowship will provide a platform for collaboration, ensuring that FAM becomes an imbedded integrated, sustainable practice across the region.
