Title: | Learning from injectable medications incidents at the end of life |
Project Description: | Mixed-methods analysis of patient safety incidents involving injectable end-of-life symptom control medication at home occurring in England and Wales, using the National Reporting and Learning System (NRLS) database of incidents reported between 2017 to 2022. NRLS patient safety incidents contain clinician reported data on events, perceived harms and free-text descriptions of what happened, why it happened and what will help prevent similar incidents in the future.
A random sample of 2000 incidents will be used to provide a quantitative description of typical safety events, including the nature of recorded harms, and enable a detailed qualitative analysis of free-text information. The findings from our study will be used to help identify specific areas for improvement and further research. |
