Project Description: | How can we improve end of life care provision for care home residents.
A project to understand what General Practitioners do when looking after care home residents at end of life, what are the gaps in care and how can it be improved. The study takes a soft systems approach to understanding how GPs look after care home residents at end of life by interviewing all those involved in the Primary Care End of Life System. By gathering these perspectives we can start to design and test measurable outcome indicators and optimise the system to deliver them.
