Title: | Using experience-based co-design to improve the impact of advance care planning on the quality of life and care of frail older people |
Project Description: | Advance care planning (ACP) has the potential to improve the quality of life and care of frail older individuals but current approaches are not working. I will use experience-based co-design (EBCD) to bring patients and staff together to collaboratively develop and refine a novel intervention to improve ACP. This will focus on ensuring both the process and outcomes of ACP reflect what is important to patients, carers and staff. |
