Summary
Heart Failure (HF) patients tell us of long-term problems. This research programme aims to address some of the key problems that HF patients identify: a feeling of helplessness when their condition deteriorates; and a lack of information and skills about many aspects of HF and HF end-of-life care. In theory, primary care takes over responsibility for the general end-of-life care needs of HF as a chronic, long-term condition. But in practice, many people fall through the net, with primary care having difficulties in identifying many continuing and end-of-life care problems, and specialist cardiology services not formally sharing responsibilities.
