Working together with patients and their families, our colleagues co-design and trial new service interventions that can better meet patients’ palliative and end of life needs in primary care. This work has two main themes: developing and evaluating a palliative care model which seeks to integrate primary and secondary care for patients living with heart failure, and developing and testing new ways for patients to manage chronic symptoms such as fatigue and breathlessness.
