Project Description: | Aim: Inform development of a PC intervention to improve uptake and adherence to SEPs in patients with PAD-IC.
Objectives: Determine the effectiveness of current PC-based exercise/activity programmes in PAD and other relevant areas (e.g. CHD, HF). Explore the reasons why uptake and completion of SEPs is poor, and why people fail to maintain physical activity levels after the SEP has finished. |
Background: | Intermittent claudication (IC) brought on by walking is often the first symptom of peripheral arterial disease (PAD). NICE recommends that a supervised exercise programme (SEP) be offered to people with IC. SEPs for IC produce short-term increases in maximal walking distance, but many people decline to take part, or do not continue to exercise beyond the supervision period, negating the long-term clinical and economic benefits of such programmes. However, little empirical evidence exists on why uptake of SEPs and adherence are so poor in people with IC.
NICE has suggested incorporating PAD-IC patients into cardiac rehabilitation classes. However, often these are delivered in a hospital environment resulting in problems with accessibility, travel and parking, indicating a need for provision in more local community/ primary care (PC) settings. |
Methodology description: | Systematic review of the effectiveness of PC-based exercise/activity programmes in PAD-IC, CHD and HF. Systematic review of qualitative literature looking at why people with PAD-IC, CAD or HF fail to engage with PC-based exercise/activity programmes; including reasons for not taking up the offer, failing to complete, and failing to remain physically active after supervision has finished. |
Outcome measure description: | These reviews will inform development of a PC-based intervention to improve uptake and completion of SEPs and longer-term physical activity in PAD. Building on expertise and research interests of Cambridge PCU, using the MRC guidelines for development of complex interventions, this work will begin development of a PC intervention to improve functional and clinical outcomes for people with PAD-IC (further funding (NIHR) will be sought). |
